I have attached another picture of how the chart should look. You are simply following the what isalready in the chart and making changes in verbage. Your job is to change what is already stated in the
chart to what I have below:
Billing and coding guidelines
Please review 90833, the following ( in BOLD )should be updated as this is not currently showing. Code
first (99202-99255) 99224-99225, 99226, 99304, 99316, 99341-99350
90838- codes have changed. The following should be updated as this is not currently showing on the
reference guide. Code first –(99202-99255) 99224-99225, 99226, 99304-99316, 99341-99350
90846 – should include the following: Service time less than 26 minutes
90847- should include the following: Service time less than 26 minutes
Under Evaluation and Management E/M codes
99202-99205 New Patient Office or Other Outpatient Services
99211-99215 Established Patient Office or Other Outpatient Services
Please make sure to put. When billing and E/M code with 90868, please make sure to use modifier 25
for the E/M code. You can bill this on the same day for all insurance companies except Medicare and
Magellan. They do not allow it to be billed on the same day.
Create a PowerPoint executing the following:
Steps to credentialing:
Slide 1. create a list of insurance carriers that credential online
Slide 2. create a list of insurance carriers that credential with a paper application
Slide 3. State the following “CAQH must be updated to include each payor you are credentialing
with because the insurance carrier will login to review the application via CAQH.”
Slide 4. State the following “register with Availity (800-282-4548) as the Administrator. When
calling, provide your company name, NPI and TIN.”
